Cache-bust favicon and apple-touch-icon URLs (v2)

Cloudflare edge cached the old placeholder bytes on the bare
/static/favicon.png and /static/apple-touch-icon.png URLs. The ?v=2
query string forces a fresh origin fetch with zero behavior change.
